My goal will always be to provide you progressive choices in web site design, choices tailored to impel each page in the direction that all leading web designs want to go. To be able to succeed in this effort, I need to know your goals from the start. It is with this in mind that I ask you to take time, time well spent, to fill out this questionnaire. Even if questions seem to be irrelevant, they all help to communicate more of a whole picture about you, your tastes and preferences, your needs and objectives, and your expectations, things that, incorporated into the design of your web site, will position you for success on the web .

This questionnaire is exactly that – a questionnaire – and does not obligate you to anything, nor does it constitute in any way a legally binding contractual document. However, neither does it constitute an agreement on my part to enter into, or substitute for, a contract when the questionnaire is submitted. Just as important to me is the confidentiality of information you provide here. What you share here, stays here, and will not be shared, given away, sold, lent out, or in any other way divulged to any other person or entity, except when authorized by YOU as needed to the design of your web site.
